Medication errors are one of the biggest global healthcare challenge harming millions of patients and costing over USD 40 billions every year worldwide. In Thailand, medication intake which is not compliant occurs over 40% at home. The 2 challenges that care providers face are :
Poor health data exchange infrastructure in developing countries make it impossible to build an effective connected care system for patients.
PharmaSafe adopted a trickle up innovative design approach to integrate medication data direct from the patients side at the point of drug dispense. With patient centric and data decenterize appoach, PharmaSafe allows medication data sharing between care provides and patients in developing countries where lacking a country-wide health data exchange infrastructure in place.
Patients will automatically receives med info, reminders and risk warnings such as allergies, drug interactions, drug duplications and more via a mobile app. The app helps to improve patients's health literacy and self care ability when they take medicine at home.
Care providers can monitor patients compliance and feedbacks in real-time with web dashboard including drug records from every point of drug dispense.

Most countries try to build a centralized patient health information system controlled by government or a central agency to provide effective connected and continuity of care to their citizens. This approach requires a fully integrated health data exchange system incorporating all care providers at every level, in both private and public sectors. This requirement is next to impossible in many countries, especially developing countries where health information infrastructure is poor and integration between private and public care providers is very hard to implement.
However, to solve this problem, PharmaSafe took a trickle up innovative design approach to integrate medication data direct from the patient side at the point of drug dispense…that is inside of the Hospital Pharmacy. Start with the right of patients to request their own medication data from care providers who are subjected to provide the data conveniently through our platform. Patients will have their medication data from any level of care providers, both public and private to self-care themselves at home on our mobile application.
With medication reminders and adverse drug risk warning system in our app, patient's home medication behavior data is integrated on to our pharmacare data platform. With this innovative approach, PharmaSafe is able to connect patients home medication behavior data with prescription data from all sources to provide the most complete patient medication 365 viewed report.
